Documentation Examples of CV:

1. RRR, no murmurs, gallops, rubs. PMI nondisplaced
2. RRR, No M/G/R
3. Irregular rhythm, S3 gallop
4. Regular rate and rhythm. No murmurs, gallops, rubs. PMI non-displaced. Carotids without bruits.
5. Regular rate and rhythm. Harsh 3/6 crescendo-decrescendo murmur heard on left sternal border. Thrill present. PMI palpated at anterior axillary line in 5^th and 6^th intercostal spaces.  Left carotid bruit present.

Documentation Examples of Peripheral Vascular:

1. No edema. Pulses 2+.
2. Warm extremities, no edema present. Calves supple, nontender. No varicosities or stasis changes. All pulses are 2+ and symmetric. No femoral or abdominal bruits.
3. Cool, pale extremities below the calf with hair loss noted. Mild erythema noted, no edema or ulcerations present. Bilateral femoral bruits, no abdominal bruits. Brachial and radial pulses 2+, DP and PT pulses 1+.
